DA Power Polishing Pad Velcro Separated

I absolutely love the DA Power system but this morning I went to go use my polishing pad to apply Black Wax on another car and the Velcro backing just fell completely off. I only washed it with some warm water and dish soap by hand. Was this an incorrect washing technique or did the pad just fail?
