pads keep coming off backing plate

i bought a g-100 a couple months ago and had no problems untill the last 2 times i used it. after paint cleaning, polishing and waxing my '01 dodge dakota about 10 times all of a sudden the pads barely hold to the backing plate, even a new pad that i only use with the bonnet to remove wax becomes way off centered then flys off the plate. it doesnt matter if i use a new or old pad and doesnt matter which product, horizontal or vertical plane, speed is always 4.5 to 5. 4-5 lbs pressure, 1 lb pressure, no pressure, no difference.

how long to backing plates last?
what speed should i be using for #83, #80, and nxt wax?
