UP or UC?
If it was UP then that explains a lot. The speed was too low too, although the pressure you used sounds about right.
To do any serious defect removal, you want UC on speed 5 and medium downward pressure with a very slow arm speed.
As a guide, push down hard on the machine until the pad stops rotating, then back off a little so it starts rotating slowly. That's about where you want to be for defect removal. And move your arm s l o w l y across the paint.
