I'm a big fan of Gorilla Glue but I don't know if I've used it for re-attaching Velcro to foam. It's a glue that expands as it cures for whatever that's worth. I have used 2-part epoxy with great success, other's have used some kind of glue used in crafts and scrap-booking? Can't think of the name.
Goop glue might work too as it's pretty tuff...

I use Auto Goop, works great so far, been run through the torture as the other pads. Ran a bead around the outside of it, none needed in the center as of yet. I have another pad that is getting "loose" in a spot or two and need to do it before too long.
